What was the biggest "fail" dish or meal you've ever cooked?

Too many to count but I do remember testing muffuletta recipes and having a surplus of olive salad. I made it into a quiche-really packed the tart shell full of olive salad and added the egg custard. It was particularly awful and Drew still likes to remind me of the olive salad quiche
